During this licensor's complaint inspections on October 20, 2022 this licensor identified that not all all providers had verification of t.b. test results or program orientation in their staff files.On October 28, during this licensor's inspection, multiple staff still did not have verification. On November 3, 2022 it was reported to this licensor that one provider still had not completed t.b. testing and was on the premises. On November 7, during an unannounced inspection, this licensor learned that a provider was on the premises with a pending background clearance.
